Nandīśvara said:— O sage, now listen to the twelve incarnations of the lord in the form of jyotirliṅgas, the cause of diverse sports.

Kedara is the snow-white snowflake and Bhima-shankara is the witch. In Varanasi the lord of the universe is Trimbaka on the bank of the Gautama river.

Vaidyanath in the citadel and Nagesh in the forest of daruka Lord Rama at the bridge and Lord Ghushmesh at the temple of Lord Śiva .

avatAradvAdazakametacchambhoH parAtmanaH ॥ sarvAnandakaraM puMsAndarzanAtsparzanAnmune॥ 5॥
O sage, the twelve incarnations of the supreme soul are wholly delightful to all men for seeing and touching.

tatrAdyassomanAtho hi candraduHkhakSayaMkaraH॥ kSayakuSThAdirogANAM nAzakaH pUjanAnmune ॥ 6॥
There, the first one, Somanātha, annihilated the distress of the moon. O sage, by worshipping, it dispels the ailments of consumption, leprosy etc.

This incarnation of Śiva, Someśa, is stationed in the auspicious land of Saurāṣṭra, in the form of a phallic image. It was worshipped by the moon formerly.

चंद्रकुण्डं च तत्रैव सर्वपापविनाशकम् ॥ तत्र स्नात्वा नरो धीमान्सर्वरोगैः प्रमुच्यते॥ ८ ॥
caMdrakuNDaM ca tatraiva sarvapApavinAzakam ॥ tatra snAtvA naro dhImAnsarvarogaiH pramucyate॥ 8 ॥
Candarakuṇḍa (the pond of the Moon) is there itself and it destroys all sins. By bathing there a man becomes intelligent and gets rid of his ailments.

सोमेश्वरं महालिंगं शिवस्य परमात्मकम्॥ दृष्ट्वा प्रमुच्यते पापाद्भुक्तिं मुक्तिं च विन्दति ॥ ९ ॥
somezvaraM mahAliMgaM zivasya paramAtmakam॥ dRSTvA pramucyate pApAdbhuktiM muktiM ca vindati ॥ 9 ॥
On seeing Someśvara, the great phallic image of Śīva, people are freed from sin. They attain worldly pleasures and salvation.

मल्लिकार्जुनसंज्ञश्चावतारश्शंकरस्य वै ॥ द्वितीयः श्रीगिरौ तात भक्ताभीष्टफलप्रदः ॥ १० ॥
mallikArjunasaMjJazcAvatArazzaMkarasya vai ॥ dvitIyaH zrIgirau tAta bhaktAbhISTaphalapradaH ॥ 10 ॥
O dear, the second incarnation of Śiva Mallikārjuna, on the mountain, Śrīśaila, is the bestower of the desires of devotees.

saMstuto liMgarUpeNa sutadarzanahetutaH ॥ gatastatra mahAprItyA sa zivaH svagirermune ॥ 11॥
O sage, Śiva went there from his mountain, with great pleasure in order to see his son Kārttikeya. It is stationed there in the form of the phallic image.

ज्योतिर्लिंगं द्वितीयन्तद्दर्शनात्पूजनान्मुने ॥ महासुखकरं चान्ते मुक्तिदन्नात्र संशयः ॥ १२॥
jyotirliMgaM dvitIyantaddarzanAtpUjanAnmune ॥ mahAsukhakaraM cAnte muktidannAtra saMzayaH ॥ 12॥
It is the second Jyotirliṅga. O sage, it, is very pleasing. By seeing and worshipping it one derives salvation hereafter. There is no doubt in this.

O dear, Śiva incarnated himself as Mahākāla in the city of Ujjayinī for the protection of his devotees.

dUSaNAkhyAsuraM yastu vedadharmapramardakam ॥। ujjayinyAM gataM vipradveSiNaM sarvanAzanam ॥ 14 ॥
The demon known as Dooshana who crushed the Vedic religion He went to Ujjaini and hated the brahmins and destroyed everything.

vedaviprasutadhyAto huGkAreNaiva sa drutam ॥ bhasmasAtkRtavAMstaM ca ratnamAla nivAsinam ॥ 15 ॥
Meditating on the son of the Vedic brāhmaṇa he quickly chanted the hook He burnt to ashes the inhabitant of Ratnamala.

तं हत्वा स महाकालो ज्योतिर्लिंगस्वरूपतः ॥ देवैस्स प्रार्थितोऽतिष्ठत्स्वभक्तपरिपालकः ॥ १६ ॥
taM hatvA sa mahAkAlo jyotirliMgasvarUpataH ॥ devaissa prArthito'tiSThatsvabhaktaparipAlakaH ॥ 16 ॥
After killing him, on being requested by the gods; lord Mahākāla stayed there in the form of a Jyotirliṅga. He continues to protect his devotees.

महाकालाह्वयं लिंगं दृष्ट्वाभ्यर्च्य प्रयत्नतः ॥ सर्वान्कामानवाप्नोति लभते परतो गतिम् ॥ १७ ॥
mahAkAlAhvayaM liMgaM dRSTvAbhyarcya prayatnataH ॥ sarvAnkAmAnavApnoti labhate parato gatim ॥ 17 ॥
By seeing and strenuously worshipping the phallic image of Mahākāla, the devotee attains his desires and the greatest goal.

oGkAraH paramezAno dhRtaH zambho parAtmanaH ॥ avatArazcaturtho hi bhaktAbhISTaphalapradaH ॥ 18 ॥
The fourth incarnation of Śiva the supreme soul, the great lord in Oṃkāra Māndhātā is a bestower of the desires of his devotees.

विधिना स्थापितो भक्त्या स्वलिंगात्पार्थिवान्मुने ॥ प्रादुर्भूतो महादेवो विन्ध्यकामप्रपूरकः ॥ १९ ॥
vidhinA sthApito bhaktyA svaliMgAtpArthivAnmune ॥ prAdurbhUto mahAdevo vindhyakAmaprapUrakaH ॥ 19 ॥
O sage, lord Śiva manifested himself from his earthen phallic image devoutly installed by Brahmā and he fulfilled the desires of the mountain.

देवैस्संप्रार्थितस्तत्र द्विधारूपेण संस्थितः ॥ भुक्तिमुक्तिप्रदो लिंगरूपो वै शक्तवत्सल ॥ २० ॥
devaissaMprArthitastatra dvidhArUpeNa saMsthitaH ॥ bhuktimuktiprado liMgarUpo vai zaktavatsala ॥ 20 ॥
On being requested by the gods the lord in the form of his phallic image stayed there two-fold, bestowing worldly pleasures and salvation and being favourably disposed towards his devotees.

praNave caiva coMkAranAmAsIlliMgamuttamam ॥ paramezvaranAmAsItpArthivazca munIzvara ॥ 21॥
The excellent phallic image abides in the syllable Oṃkāra. O great sage, it also abides in the earthen phallic image in Oṃkāra Māndhātā.

भक्ताभीष्टप्रदो ज्ञेयो योपि दृष्टोर्चितो मुने ॥ ज्योतिर्लिंगे महादिव्ये वर्णिते ते महामुने॥ २२॥
bhaktAbhISTaprado jJeyo yopi dRSTorcito mune ॥ jyotirliMge mahAdivye varNite te mahAmune॥ 22॥
O sage, O great sage, whatever is seen or worshipped ill the extremely divine phallic image described to you is the bestower of the desires of all devotees.

केदारेशोवतारस्तु पंचमः परमश्शिवः ॥ ज्योतिर्लिंगस्वरूपेण केदारे संस्थितस्य च ॥ २३ ॥
kedArezovatArastu paMcamaH paramazzivaH ॥ jyotirliMgasvarUpeNa kedAre saMsthitasya ca ॥ 23 ॥
The fifth great incarnation of Śiva is the Kedāreśā. He is stationed in Kedāra in the form of Jyotirliṅga.

नरनारायणाख्यौ याववतारौ हरेर्मुने ॥ तत्प्रार्थितश्शिवस्तत्स्थैः केदारे हिमभूधरे ॥ २४ ॥
naranArAyaNAkhyau yAvavatArau harermune ॥ tatprArthitazzivastatsthaiH kedAre himabhUdhare ॥ 24 ॥
It was at the request of the twofold incarnations of Viṣṇu, named Nara and Nārāyaṇa that Śiva stayed in Kedāra on the mountain Himavat, O sage.

ताभ्यां च पूजितो नित्यं केदारेश्वरसंज्ञकः ॥ भक्ताभीष्टप्रदः शम्भुर्दर्शनादर्चनादपि ॥ २५॥
tAbhyAM ca pUjito nityaM kedArezvarasaMjJakaH ॥ bhaktAbhISTapradaH zambhurdarzanAdarcanAdapi ॥ 25॥
Śiva named Kedāreśvara was worshipped everyday by them. He is the bestower of the desires of the devotees, by his sight and worship.

अस्य खण्डस्य स स्वामी सर्वेशोपि विशेषतः ॥ सर्वकामप्रदस्तात सोवतारश्शिवस्य वै ॥ २६॥
asya khaNDasya sa svAmI sarvezopi vizeSataH ॥ sarvakAmapradastAta sovatArazzivasya vai ॥ 26॥
Although he is the lord of all, he is the particular reigning deity of this territory. O dear that incarnation of Śiva is the bestower of all desires.

भीमशंकरसंज्ञस्तु षष्ठः शम्भोर्महाप्रभोः ॥ अवतारो महालीलो भीमासुरविनाशनः ॥ २७॥
bhImazaMkarasaMjJastu SaSThaH zambhormahAprabhoH ॥ avatAro mahAlIlo bhImAsuravinAzanaH ॥ 27॥
The sixth incarnation of Śiva, the great lord named Bhīmaśaṅkara, performed great sports. He destroyed the demon Bhīma.

सुदक्षिणाभिधम्भक्तङ्कामरूपेश्वरन्नृपम्॥ यो ररक्षाद्भुतं हत्वासुरन्तं भक्तदुःखदम् ॥ २८॥
sudakSiNAbhidhambhaktaGkAmarUpezvarannRpam॥ yo rarakSAdbhutaM hatvAsurantaM bhaktaduHkhadam ॥ 28॥
After killing this wonderfully lusty demon who caused misery to the devotees, the lord saved the devotee Sudakṣiṇa who was the king of Kāmarūpa.

bhImazaGkaranAmA sa DAkinyAM saMsthitassvayam ॥ jyotirliMgasvarUpeNa prArthitastena zaMkaraH ॥ 29॥
Requested by him Śiva himself in the name of Bhimaśaṅkara stayed in Ḍākinī in the form of Jyotirliṅga.

विश्वेश्वरावतारस्तु काश्यां जातो हि सप्तमः ॥ सर्वब्रह्माण्डरूपश्च भुक्तिमुक्तिप्रदो मुने ॥ ३० ॥
vizvezvarAvatArastu kAzyAM jAto hi saptamaH ॥ sarvabrahmANDarUpazca bhuktimuktiprado mune ॥ 30 ॥
O sage, the seventh incarnation as Viśveśvara at Kāśī is in the form of the entire cosmic egg. It yields worldly pleasures and salvation.

पूजितस्सर्वदेवैश्च भक्त्या विष्ण्वादिभिस्सदा ॥ कैलासपतिना चापि भैरवेणापि नित्यशः ॥ ३१ ॥
pUjitassarvadevaizca bhaktyA viSNvAdibhissadA ॥ kailAsapatinA cApi bhairaveNApi nityazaH ॥ 31 ॥
He is always worshipped by Viṣṇu and all other gods as well as Kubera and Bhairava.

ज्योतिर्लिंगस्वरूपेण संस्थितस्तत्र मुक्तिदः ॥ स्वयं सिद्धस्वरूपो हि तथा स्वपुरि स प्रभुः ॥ ३२ ॥
jyotirliMgasvarUpeNa saMsthitastatra muktidaH ॥ svayaM siddhasvarUpo hi tathA svapuri sa prabhuH ॥ 32 ॥
He is stationed there in the form of Jyotirliṅga. In his own city the lord is in the form of Siddha and he confers salvation.

काशीविश्वेशयोर्भक्त्या तन्नामजपकारकाः ॥ निर्लिप्ताः कर्मभिर्न्नित्यं केवल्यपदभागिनः ॥ ३३ ॥
kAzIvizvezayorbhaktyA tannAmajapakArakAH ॥ nirliptAH karmabhirnnityaM kevalyapadabhAginaH ॥ 33 ॥
Those who repeat the names of Kāśī and Viśvśa and perform their Japa with devotion become unaffected by actions and reach the state of Kaivalya.

त्र्यंबकाख्योऽवतारो यः सोष्टमो गौतमीतटे ॥ प्रार्थितो गौतमेनाविर्बभूव शशिमौलिनः ॥ ३४॥
tryaMbakAkhyo'vatAro yaH soSTamo gautamItaTe ॥ prArthito gautamenAvirbabhUva zazimaulinaH ॥ 34॥
The eighth incarnation Tryambaka of the moon-crested lord Śiva made its appearance on the banks of the river Gautamī on being requested by the sage Gautama.

गौतमस्य प्रार्थनया ज्योतिर्लिंग स्वरूपतः ॥ स्थितस्तत्राचलः प्रीत्या तन्मुनेः प्रीतिकाम्यया॥ ३५॥
gautamasya prArthanayA jyotirliMga svarUpataH ॥ sthitastatrAcalaH prItyA tanmuneH prItikAmyayA॥ 35॥
At the request of Gautama and with a desire to delight that sage, the lord stood there motionless in the form of Jyotirliṅga.

तस्य सन्दर्शनात्स्पर्शाद्दर्शनाच्च महेशितुः ॥ सर्वे कामाः प्रसिध्यन्ति ततो मुक्तिर्भवेदहो ॥ ३६॥
tasya sandarzanAtsparzAddarzanAcca mahezituH ॥ sarve kAmAH prasidhyanti tato muktirbhavedaho ॥ 36॥
All desires fructify at the sight and touch of the great lord. Thereafter salvation too is attained.

शिवानुग्रहतस्तत्र गंगा नाम्ना तु गौतमी ॥ संस्थिता गौतमप्रीत्या पावनी शंकरप्रिया ॥ ३७॥
zivAnugrahatastatra gaMgA nAmnA tu gautamI ॥ saMsthitA gautamaprItyA pAvanI zaMkarapriyA ॥ 37॥
With the blessings of Śiva, and pleased with Gautama, the holy river Gaṅgā, the beloved of Śiva, stayed there in the name of Gautamī.

वैद्यनाथावतारो हि नवमस्तत्र कीर्तितः ॥ आविर्भूतो रावणार्थं बहुलीलाकरः प्रभुः॥ ३८॥
vaidyanAthAvatAro hi navamastatra kIrtitaH ॥ AvirbhUto rAvaNArthaM bahulIlAkaraH prabhuH॥ 38॥
The ninth incarnation is glorified as Vaidyanātha. The lord indulging in many sports manifested himself for the sake of Rāvaṇa.

तदानयनरूपं हि व्याजं कृत्वा महेश्वरः ॥ ज्योतिर्लिंगस्वरूपेण चिताभूमौ प्रतिष्ठितः ॥ ३९॥
tadAnayanarUpaM hi vyAjaM kRtvA mahezvaraH ॥ jyotirliMgasvarUpeNa citAbhUmau pratiSThitaH ॥ 39॥
Under the pretext of fetching him, lord Śiva was installed in the cremation ground in the form of Jyotirliṅga.

The lord in the name of Vaidyanātha became famous in the three worlds. Seeing and worshipping him with devotion yields worldly pleasures and salvation.

The incarnation of Nāgeśvara is glorified as the tenth one. The lord manifested himself as the chastiser of the wicked for his devotees.

He who reads this always or listens to it devotedly attains all desires and then becomes liberated, to be sure.
